Three Oaks Health Services’ goal is to create an environment that affords our residents the opportunity to be a partner in the care they receive, to develop trusting relationships with our caregivers, and to reach their highest level of independence. We have been serving Marshfield and surrounding communities since 2015. We are located in a quiet setting on the north edge of Marshfield near the park. We are proud to be active members of the Marshfield Chamber of Commerce. In a recent customer satisfaction survey, we are proud to share, that 100% of our residents indicated their overall experience was good to great and would choose Three Oaks again if the need arose. We are also proud to be on the U.S. News & World Report Best Nursing Home list for 2018.

“I am passionate about the skilled nursing setting because it allows us, as therapists, to make connections across the continuum of care. We treat the person in the skilled nursing facility for an inpatient stay, and through extra training received and community connections we have built, we then have the unique ability to continue to follow them as their needs change. After the skilled nursing stay, we can follow our patients with home health, outpatient rehab at the skilled nursing facility, or outpatient rehab at the assisted living facilities if needed. This allows us to build strong relationships with our clientele and achieve excellent rehab outcomes.”
