As a member of the North Shore Healthcare family, our center’s dedicated healthcare professionals strive to deliver quality care to those who have chosen us. Three Oaks Health Services’ goal is to create an environment that affords our residents the opportunity to be a partner in the care they receive, to develop trusting relationships with our caregivers, and to reach their highest level of independence. We are passionate about making our center The Right Choice for you or your loved one.

We have been serving Marshfield and surrounding communities since 2015. We are located in a quiet setting on the north edge of Marshfield near the park. We are proud to be active members of the Marshfield Chamber of Commerce.

In a recent customer satisfaction survey, we are proud to share, that 100% of our residents indicated their overall experience was good to great and would choose Three Oaks again if the need arose. We are also proud to be on the U.S. News & World Report Best Nursing Home list for 2018.

We recently were awarded a 2019 Bronze – Commitment to Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). The Award Program has three levels: Bronze, Silver, and Gold. As a recipient of the Bronze – Commitment to Quality Award, we may now move forward in developing approaches and achieving performance levels that meet the Silver – Achievement in Quality Award criteria.

“I am passionate about the skilled nursing setting because it allows us, as therapists, to make connections across the continuum of care. We treat the person in the skilled nursing facility for an inpatient stay, and through extra training received and community connections we have built, we then have the unique ability to continue to follow them as their needs change. After the skilled nursing stay, we can follow our patients with home health, outpatient rehab at the skilled nursing facility, or outpatient rehab at the assisted living facilities if needed. This allows us to build strong relationships with our clientele and achieve excellent rehab outcomes.”
